Direct Support Professional (DSP)
Twin Cities Metro, MN
Provide hands-on support to individuals with developmental disabilities in their homes and communities. Help clients build daily living skills, achieve personal goals, and participate fully in community life.
What’s Included

- Reliable transportation

- CPR/First Aid certification (or willingness to obtain)
Preferred
- Experience working with individuals with disabilities

Care Coordinator
New Brighton, MN (Hybrid)
Develop and monitor individualized support plans for clients. Collaborate with families, county agencies, and healthcare providers to ensure comprehensive, person-centered care.
Requirements
- Bachelor's degree in Social Work, Human Services, or related field
- 2+ years experience in disability services or case management
- Knowledge of Minnesota DHS waiver programs
- Strong organizational and communication skills
- Valid driver's license
Preferred
- Experience with BI, CAC, CADI, or DD waivers
- Bilingual (Somali, Spanish, or other languages)
- Certified Case Manager (CCM) credential
Employment Specialist
Twin Cities Metro, MN
Support individuals with disabilities in finding and maintaining competitive integrated employment. Develop relationships with employers, provide job coaching, and facilitate vocational rehabilitation services.
